Used For:
- Ofloxacin is a fluoroquinolone antibiotic used to treat a wide range of bacterial infections caused by both Gram-negative and Gram-positive bacteria. It works by inhibiting bacterial DNA synthesis, leading to bacterial cell death.
- Common indications include:
- Urinary tract infections (UTIs), including cystitis and pyelonephritis.
- Respiratory tract infections, including bronchitis and pneumonia.
- Skin infections such as cellulitis.
- Sexually transmitted infections (STIs), including gonorrhea.
- Eye infections, including conjunctivitis (when used in eye drop form).
- Intra-abdominal infections like peritonitis and diverticulitis.
- Prostatitis (infection of the prostate gland).

Common Side Effects:
- Nausea, diarrhea, abdominal pain, or upset stomach.
- Headache, dizziness, or lightheadedness.
- Rash or itching.

Serious Side Effects:
- Tendon rupture or tendonitis (especially in older adults or those taking corticosteroids).
- Central nervous system effects, including confusion, hallucinations, or seizures.
- QT interval prolongation, which can lead to serious heart arrhythmias.
- Peripheral neuropathy (nerve damage), which can cause numbness, tingling, or burning sensations in the hands or feet.
- Severe allergic reactions such as anaphylaxis, difficulty breathing, swelling of the face or throat.
- Liver problems, such as jaundice (yellowing of the skin or eyes) or abnormal liver function tests.
- Clostridium difficile-associated diarrhea (C. diff), which can cause severe diarrhea and intestinal damage.
- Other Considerations: Some people may experience sun sensitivity (photosensitivity), which increases the risk of sunburn.
Precautions:
- Tendon Damage: Ofloxacin can increase the risk of tendonitis and tendon rupture, especially in older adults and those on corticosteroids. Patients should report any pain, swelling, or inflammation in tendons, particularly in the Achilles tendon.
- Neurological Effects: Ofloxacin may cause central nervous system disturbances, such as dizziness, confusion, or seizures. It should be used with caution in individuals with a history of seizures or other neurological disorders.
- Heart Issues: Use with caution in patients with a history of heart problems or arrhythmias, as it can prolong the QT interval and increase the risk of abnormal heart rhythms.
- Liver and Kidney Function: The drug is metabolized by the liver and excreted by the kidneys, so it should be used with caution in individuals with liver or kidney impairment. Dosage adjustments may be needed in these patients.
- Pregnancy and Breastfeeding: Ofloxacin is classified as Category C for pregnancy, meaning it should only be used during pregnancy if the benefits outweigh the risks. It is excreted in breast milk, so breastfeeding mothers should consult their doctor before using it.
- Sun Exposure: Ofloxacin can cause photosensitivity, so patients should avoid direct sunlight or use sunscreen and protective clothing while taking this medication.
