Used For:

This combination is a broad-spectrum antibiotic used to treat severe bacterial infections.

  • Imipenem (Carbapenem antibiotic):

    • Kills bacteria by disrupting cell wall synthesis.
    • Effective against Gram-positive & Gram-negative bacteria, including some drug-resistant strains.

  • Cilastatin (DHP-1 Inhibitor):

    • Prevents the breakdown of Imipenem in the kidneys.
    • Increases the effectiveness of Imipenem.
  • Pneumonia (hospital-acquired, ventilator-associated)
  • Urinary tract infections (complicated UTIs)
  • Intra-abdominal infections (peritonitis, abscesses)
  • Skin & soft tissue infections
  • Sepsis
  • Bone & joint infections

Side Effects:

  • Common:

    • Nausea, vomiting, diarrhea
    • Headache
    • Rash, itching
    • Injection site pain

  • Serious (but rare):

    • Seizures (higher risk in kidney disease or CNS disorders)
    • Severe allergic reactions (swelling, breathing difficulty)
    • Liver toxicity (yellowing of skin/eyes)
    • Clostridium difficile infection (severe diarrhea)

Precautions:

  • Adjust dose in kidney disease—risk of seizures increases.
  • Not recommended for CNS disorders (e.g., epilepsy) due to seizure risk.
  • Monitor liver & kidney function regularly.
  • Avoid use in penicillin or carbapenem allergy.
