Used For:
- Organ Transplant Rejection Prevention: Sirolimus is primarily prescribed to prevent kidney transplant rejection. It may also be used off-label for other organ transplants in some cases.
- Certain Rare Diseases: Sirolimus may be prescribed for rare disorders like lymphangioleiomyomatosis (LAM), a lung disease that primarily affects women.
- Cancer: Because it inhibits cell growth, sirolimus is being studied as a potential treatment for certain cancers and as a part of cancer therapies.
Side Effects:
Common side effects include:
- Mouth ulcers or sores
- Increased risk of infections due to immunosuppression
- Elevated cholesterol and triglycerides in the blood
- Nausea, diarrhea, and stomach discomfort
- Acne or rash
- Anemia and other blood disorders
- Swelling in the arms, legs, or other areas
- Delayed wound healing
Serious side effects include:
- Increased risk of infections: Sirolimus can significantly weaken the immune system, making infections more likely.
- Lung issues: Such as interstitial lung disease, which can cause difficulty breathing, cough, or chest pain.
- Kidney issues: Although sirolimus is used in kidney transplants, it can still cause or worsen kidney problems, especially when used with other immunosuppressants.
- Liver dysfunction: It may cause elevated liver enzymes and, in rare cases, liver damage.
- Lymphomas and other cancers: Long-term use may increase the risk of developing certain cancers due to its immunosuppressive effects.
Precautions:
- Infection risk: Since sirolimus suppresses the immune system, patients should take extra precautions to avoid infections, practice good hygiene, and promptly report any signs of infection to their healthcare provider.
- Kidney and liver function monitoring: Regular blood tests are needed to monitor kidney and liver function, as well as sirolimus blood levels, to ensure appropriate dosing and avoid toxicity.
- Avoid live vaccines: Live vaccines should not be administered to patients on sirolimus, as their immune system may not be able to handle it.
- Pregnancy and breastfeeding: Sirolimus is generally not recommended during pregnancy, as it may harm a developing fetus. Women should use effective contraception while on this medication. Its safety during breastfeeding is unknown, so it’s advised to avoid breastfeeding while on sirolimus.
