Used For:

  • Type 2 Diabetes Mellitus (adjunct to diet and exercise)

  • Chronic Weight Management (in adults with BMI ≥27 with comorbidities or BMI ≥30)

  • May reduce cardiovascular risk in diabetic patients

Side Effects:

  • Very Common (≥10%):

    • Nausea/vomiting

    • Diarrhea/constipation

    • Abdominal pain

  • Common (1-10%):

    • Decreased appetite

    • Headache

    • Fatigue

    • Hypoglycemia (when used with insulin/sulfonylureas)

  • Serious (Rare):

    • Pancreatitis

    • Diabetic retinopathy complications

    • Gallbladder disorders

    • Acute kidney injury

Precaution:

  • Contraindicated in:

    • Type 1 diabetes

    • Diabetic ketoacidosis

    • Personal/family history of medullary thyroid carcinoma

    • Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)

  • Requires gradual dose escalation (0.25mg→0.5mg→1mg)

  • Administer subcutaneously once weekly (any time of day)

  • Rotate injection sites (abdomen, thigh, or upper arm)

  • Monitor for suicidal thoughts/behavior (weight loss indication)

  • Pregnancy Category C (use only if potential benefit justifies risk)
