Used For:

1. Malaria Treatment and Prevention:

   • Specifically effective against the liver stages of Plasmodium vivax and Plasmodium ovale to prevent relapses (radical cure).
   • Used as part of a treatment plan for malaria eradication when combined with other antimalarial drugs.

2. Gametocidal Activity:

   • Kills gametocytes (sexual forms of malaria parasites) of Plasmodium falciparum, reducing transmission to mosquitoes.

3. Pneumocystis jirovecii Pneumonia (PCP):

   • Occasionally used as an off-label treatment in combination with other medications.

Side Effects:

1. Common Side Effects:

   • Nausea
   • Vomiting
   • Stomach pain or cramps

2. Serious Side Effects:

   • Hemolytic anemia (especially in people with G6PD deficiency)
   • Methemoglobinemia (elevated levels of methemoglobin in the blood, causing cyanosis)
   • Jaundice
   • Severe abdominal pain

Precautions:

1. G6PD Deficiency Testing:

   • Mandatory before starting primaquine, as it can cause life-threatening hemolysis in individuals with glucose-6-phosphate dehydrogenase deficiency.

2. Pregnancy and Breastfeeding:

   • Not recommended during pregnancy unless G6PD status of the fetus is known (rarely determined).
   • Caution while breastfeeding unless both mother and infant have been tested for G6PD deficiency.

3. Drug Interactions:

   • Use cautiously with medications that also increase the risk of hemolysis or methemoglobinemia.
   • Avoid with other myelosuppressive drugs.

4. Underlying Conditions:

   • Avoid in individuals with severe blood disorders or autoimmune conditions causing hemolysis.
