Used For:

Pembrolizumab is approved for the treatment of various cancers, including:

  1. Non-Small Cell Lung Cancer (NSCLC):

    • Used alone or in combination with chemotherapy in advanced or metastatic cases.
  2. Melanoma:

    • For advanced or unresectable melanoma, often used when other treatments have failed.
  3. Head and Neck Cancer:

    • For certain cases of recurrent or metastatic squamous cell carcinoma of the head and neck.
  4. Classical Hodgkin Lymphoma:

    • Used for patients who have relapsed or refractory disease after prior treatments.
  5. Urothelial Carcinoma:

    • For advanced bladder cancer or cancer of the urinary tract.
  6. Other Cancers:

    • Pembrolizumab is also used to treat cancers such as cervical cancer, esophageal cancer, gastric cancer, and renal cell carcinoma, among others.

Side Effects:

  1. Common Side Effects:

    • Fatigue
    • Rash
    • Diarrhea or colitis (inflammation of the colon)
    • Nausea
    • Loss of appetite
    • Cough
    • Shortness of breath
  2. Serious Side Effects:

    • Immune-related side effects: Since pembrolizumab enhances immune activity, it can trigger inflammation in various organs, leading to:
      • Pneumonitis (inflammation of the lungs)
      • Colitis (inflammation of the colon)
      • Hepatitis (inflammation of the liver)
      • Nephritis (inflammation of the kidneys)
      • Endocrinopathies (disorders of hormone-producing glands, such as the thyroid or adrenal glands)
    • Severe allergic reactions (e.g., rash, itching, difficulty breathing)
    • Infusion-related reactions (e.g., fever, chills, low blood pressure)
  3. Long-Term Side Effects:

    • Chronic inflammation: Persistent immune responses may lead to long-term issues with various organs.

Precautions:

  1. Immune System Disorders:

    • Pembrolizumab can cause autoimmune reactions where the immune system attacks healthy tissues. Patients with autoimmune diseases or a history of organ transplant should discuss the use of pembrolizumab with their oncologist.
  2. Pregnancy and Breastfeeding:

    • Pembrolizumab can harm a developing fetus, so it is contraindicated during pregnancy. Effective contraception should be used during treatment and for at least 4 months after the last dose. It is not recommended during breastfeeding.
  3. Liver and Kidney Function:

    • Pembrolizumab can affect liver and kidney function, so regular monitoring of these organs is essential during treatment.
  4. Infection Risk:

    • While it stimulates the immune system to fight cancer, it may also increase the risk of infections, particularly in people with weakened immune systems.
