Used For:

Montelukast is a medication primarily used for the treatment of asthma and allergic conditions. It works by blocking leukotrienes, chemicals in the body that cause inflammation and constriction of the airways.

It is commonly prescribed for the following conditions:

  1. Asthma: To prevent asthma attacks in both adults and children (often used as a maintenance therapy to control symptoms, but not to treat acute attacks).
  2. Allergic Rhinitis: To relieve symptoms of hay fever, such as sneezing, runny nose, and itchy eyes caused by seasonal or year-round allergies.
  3. Exercise-Induced Bronchoconstriction: To prevent asthma symptoms that are triggered by exercise, especially in individuals with exercise-induced bronchoconstriction (EIB).
  4. Chronic Allergic Rhinitis: To manage symptoms of perennial (year-round) allergies, like nasal congestion, sneezing, and itching.

Side Effects:

While montelukast is generally well tolerated, some individuals may experience side effects, including:

  • Common Side Effects:
    • Headache
    • Stomach pain
    • Diarrhea
    • Cough
    • Fever
    • Nasal congestion or sinus infections
    • Throat irritation
  • Serious Side Effects (though rare):
    • Behavioral changes: Anxiety, depression, agitation, aggression, mood swings, or thoughts of self-harm (especially in children and adolescents).

    • Sleep disturbances: Insomnia or vivid dreams.

    • Allergic reactions: Rash, itching, swelling (especially of the face/tongue/throat), or trouble breathing, which could indicate a serious allergic reaction (angioedema).

    • Liver problems: Uncommon, but signs of liver injury (e.g., yellowing of the skin/eyes, dark urine, persistent nausea or abdominal pain).

    • Eosinophilic conditions: Rarely, montelukast has been associated with eosinophilic conditions (eosinophilic granulomatosis with polyangiitis), which involves inflammation of blood vessels and can lead to serious complications.

Precautions:

Montelukast should be used with caution in certain individuals, and a healthcare provider should be consulted before starting treatment, especially if any of the following apply:

  • Allergic reactions: If you are allergic to montelukast or any of its ingredients, you should avoid it.
  • Mental health: Individuals with a history of depression, anxiety, or other mental health conditions should be monitored closely, as montelukast may have psychiatric side effects, such as mood changes, irritability, and suicidal thoughts.
  • Liver disease: If you have liver problems, your doctor may need to monitor your liver function during treatment.
  • Pregnancy and breastfeeding: Montelukast is classified as a pregnancy category B medication, meaning it is generally considered safe during pregnancy, but should be used only if clearly needed. It is also excreted in breast milk, so you should discuss with your doctor if you're breastfeeding.
  • Other medications: Montelukast may interact with other medications, particularly those affecting the liver or those that are metabolized by the cytochrome P450 system (e.g., certain anticonvulsants, rifampin). Make sure your healthcare provider knows about all other medications you're taking.
