Used For:

  • Chloramphenicol is a broad-spectrum antibiotic used to treat a variety of bacterial infections. It works by inhibiting protein synthesis in bacteria, leading to their death. It is typically reserved for serious infections due to its potential side effects.
  • Common Indications include:
    • Meningitis, especially caused by Haemophilus influenzae or Neisseria meningitidis.
    • Typhoid fever and paratyphoid fever (caused by Salmonella typhi).
    • Bacterial eye infections, including conjunctivitis (as topical eye drops or ointment).
    • Intra-abdominal infections, such as peritonitis or abscesses.
    • Respiratory tract infections caused by susceptible organisms.

Side Effects:

  • Common Side Effects:
    • Nausea, vomiting, or diarrhea.
    • Rash or other skin reactions.
    • Headache or dizziness.
    • Stomach discomfort or indigestion.
  • Serious Side Effects:
    • Aplastic anemia (a rare but life-threatening condition where the bone marrow stops producing blood cells). This is one of the most serious side effects and can occur after prolonged or high-dose use.
    • Gray baby syndrome: This is a severe and potentially fatal reaction that can occur in newborns due to their inability to metabolize the drug properly. It results in cyanosis (blue skin), hypothermia, vomiting, and cardiovascular collapse.
    • Bone marrow suppression, which can lead to conditions like leukopenia (low white blood cells) and thrombocytopenia (low platelets), increasing the risk of infection and bleeding.
    • Liver toxicity: Rare but serious liver damage, indicated by jaundice or elevated liver enzymes.
    • Peripheral neuropathy, causing numbness or tingling, particularly with prolonged use.

Precautions:

  • Bone Marrow Suppression: Chloramphenicol should be used cautiously in patients with existing bone marrow disorders or those who are pregnant (especially in the third trimester) or breastfeeding, as it can cause bone marrow suppression.
  • Kidney and Liver Disease: Caution should be exercised in patients with liver or kidney disease, as these conditions can impair the body’s ability to clear the drug, leading to toxicity.
  • Pregnancy and Breastfeeding: Chloramphenicol is generally considered contraindicated during pregnancy, particularly in the third trimester and in newborns due to the risk of gray baby syndrome. It is also excreted in breast milk, so it should be avoided in nursing mothers unless absolutely necessary.
  • Infection Risk: Since chloramphenicol can suppress bone marrow function, blood counts should be monitored regularly, especially during prolonged use.
  • Eye Infection: When used topically as eye drops or ointment, it may cause mild local irritation, including stinging or redness, but severe allergic reactions are rare.
