Used For:
- Treatment-Resistant Depression: Effective for cases of major depressive disorder (MDD) that do not respond to other antidepressants alone.
- Bipolar Depression: Approved for the depressive episodes of bipolar disorder, particularly where other mood stabilizers may be less effective.
- Schizoaffective Disorder (Off-Label): May be used to treat mood symptoms associated with schizoaffective disorder, although not formally approved for this use.
Side Effects:

Common Side Effects:
- Olanzapine: Weight gain, drowsiness, dry mouth, constipation, and increased appetite.
- Fluoxetine: Nausea, headache, insomnia, sexual dysfunction, and anxiety, particularly when starting the medication.

Less Common but Serious Side Effects:
- Metabolic Changes (Olanzapine): Increased blood glucose, cholesterol, and triglycerides, which can lead to diabetes or metabolic syndrome.
- Serotonin Syndrome (Fluoxetine): A rare but serious condition caused by excess serotonin, leading to agitation, confusion, rapid heart rate, and high blood pressure.
- Neuroleptic Malignant Syndrome (NMS) (Olanzapine): A rare, potentially life-threatening reaction with symptoms like muscle rigidity, fever, and altered mental status.
- Suicidal Thoughts (Fluoxetine): Like all SSRIs, fluoxetine may increase the risk of suicidal thinking in adolescents and young adults.
Precautions:
- Metabolic Monitoring: Due to olanzapine’s potential for metabolic side effects, regular monitoring of weight, blood glucose, and cholesterol is essential.
- Avoid in Elderly Dementia Patients: Olanzapine increases the risk of death in elderly dementia patients and should be used cautiously in this population.
- Avoid Alcohol: Alcohol can intensify side effects such as drowsiness and dizziness.
- Tapering: Fluoxetine, if stopped, should be tapered to avoid withdrawal symptoms, although it has a longer half-life than many SSRIs, making withdrawal milder.
- Pregnancy and Breastfeeding: This combination is typically avoided in pregnancy and breastfeeding unless benefits clearly outweigh risks, as both medications can affect the fetus or infant.
