Used For:
- Autoimmune Diseases: Commonly used for conditions like rheumatoid arthritis, lupus, Crohn’s disease, ulcerative colitis, and myasthenia gravis.
- Organ Transplantation: Helps prevent organ rejection by suppressing the immune system’s response against the transplanted organ.
- Other Conditions: Sometimes used off-label for other inflammatory conditions where immune suppression is beneficial.
Side Effects:
- Common Side Effects: Nausea, vomiting, diarrhea, and mild fatigue.

Less Common but Serious Side Effects:
- Bone Marrow Suppression: Azathioprine can lower blood cell counts (white cells, red cells, and platelets), increasing the risk of infections, anemia, and bleeding.
- Liver Toxicity: It can cause elevated liver enzymes and, rarely, severe liver damage. Monitoring liver function is essential.
- Increased Cancer Risk: Long-term use is associated with a slightly increased risk of certain cancers, such as skin cancers and lymphomas.
- Pancreatitis: Rarely, it can cause inflammation of the pancreas, which may present with severe abdominal pain.
Precautions:
- Regular Blood Monitoring: Due to risks of bone marrow suppression and liver toxicity, regular blood tests are required to monitor blood counts and liver function.
- Avoid Live Vaccines: Since it weakens the immune system, live vaccines (e.g., MMR, varicella) should generally be avoided.
- Sun Protection: Since azathioprine increases the risk of skin cancers, sun protection and regular skin checks are recommended.
- Drug Interactions: Certain medications, like allopurinol (for gout), can increase the effects of azathioprine, necessitating dose adjustments.
