Used For

  • Angina Pectoris: Nicorandil is used to prevent and treat stable angina, helping to relieve chest pain and improve exercise tolerance.
  • Heart Failure: It may be used as an adjunct treatment in patients with heart failure due to its vasodilatory effects.

Nicorandil has a dual mechanism:

  • Vasodilator Effect: It relaxes and dilates blood vessels, which reduces the workload on the heart and improves blood supply to the heart muscle.
  • Potassium Channel Activator: It activates ATP-sensitive potassium channels, which also helps in vasodilation.

Side Effects

Common side effects include:

  • Headache: This is a frequent side effect due to vasodilation.
  • Dizziness or lightheadedness: Particularly when standing up quickly.
  • Nausea or gastrointestinal disturbances.
  • Flushing: A feeling of warmth or redness in the skin.

Less common but serious side effects can include:

  • Severe hypotension: Abrupt drops in blood pressure can occur, especially when combined with other medications.
  • Angina exacerbation: In some cases, angina symptoms may worsen.
  • Skin Reactions: Rarely, it may cause skin lesions or ulceration, especially with prolonged use.

Precautions

  • Blood Pressure: Monitor blood pressure regularly, as nicorandil can cause hypotension.
  • Concurrent Medications: Use caution when combined with other vasodilators or medications that lower blood pressure.
  • Heart Conditions: Patients with severe heart conditions should be monitored closely.
  • Pregnancy and Breastfeeding: Consult a healthcare provider before use, as safety during pregnancy and lactation is not fully established.
  • Gastrointestinal Conditions: Use cautiously in patients with a history of gastrointestinal ulcers, as nicorandil may increase the risk of ulceration.
